HMO (Health Maintenance Options) 2011
The preventative medicine health care package allows horse owners a way to save money, while providing for routine health care for an entire year. We continue to offer three options.
Option I: This is the most comprehensive package. Option 1A covers all normal vaccinations, including an additional mid-summer flu/rhino respiratory vaccination, a complimentary dental exam, a yearly Coggins test, and a complete blood profile analysis. A thorough physical examination is conducted: this includes temperature, heart and lung auscultation, skin evaluation, internal/external eye examination, leg palpation, and examination of the way of going on a lead line. We also include two fecal flotations. Option I is designed to offer complete preventative medical protection for your horse while also providing information regarding the internal health of such body systems as the blood, liver and kidney. Option 1B includes everything listed under Option 1A as well as 1 Botulism vaccine.
Horses enrolled in Option 1 receive a 10% discount off of all veterinary services (provided by Coastal Equine), including farm calls, provided that they remain in current financial status throughout the year. Delinquent accounts will have their 10% discount privileges revoked.
Option II: The Option 2A package includes routine vaccinations, a yearly Coggins test, plus a complimentary dental examination. This option provides solid preventative care minus the depth of blood or physical evaluations. This is the current standard level of care for horses in Hampton Roads and will fulfill the specifications for most horse transport or horse show requirements. Option 2B includes all of the above plus one Botulism vaccine.
Option III: The Option 3A package provides the minimal level of solid preventative care to horses in this area. Option 3 horses are only vaccinated against the major, life-threatening diseases. Please note that other disease vaccinations are not included in this plan. A yearly Coggins test and a complimentary dental examination are also included. Option 3B includes all the above plus one Botulism vaccine.
Conditions of Enrollment
- Payment for the preventive health care packages is required at or before the time of service. THERE IS NO BILLING FOR HMO OPTIONS. This is true of individual horse owners as well as those in a large boarding barn. You may send in a payment in advance, call in a credit card authorization, or have your payment waiting at your specific spring appointment. Remember that the deadline for enrollment is April 15th. If your appointment is scheduled later than April 15th, you will have to send in an advance payment for your selected Option. We will not retroactively apply an HMO to spring services previously rendered.
- Client accounts must be paid in full prior to enrollment in any preventative health care Option. If your payment is for an HMO plan as well as payment on your account, please provide a clear written explanation of what the monies are allocated for.
- Farm calls are not included in the price of the health care options and there is a minimum farm call to all clients regardless of boarding circumstances or location. Split farm calls are pro-rated based on the number of horses per call. Please inquire about what the farm call is for your location and the number of horses to be seen.
- Coastal Equine is not responsible for ensuring that enrolled horses receive their required vaccinations, etc. The horse owner is responsible to make the appointment for services or sign up at their barn during the scheduled vaccination day.
- If you do not utilize all the services included in your selected Option, there is no refund or substitutions made on your unused opportunities (use it or lose it).
- Selected Options may not be changed during the enrollment year, but may be changed prior to another enrollment.
- If a horse is sold, or dies while enrolled in one of the health care packages, a pro-rated refund will be given, based on the remaining services. You can also transfer the program to another horse.
- If an Option client does not remain in current financial status, remaining Option services (monies) may be utilized to make the account current.
The enrollment period is from January 1, 2011 to April 15, 2011. The preventative medicine health care plan begins with receipt of payment prior to April 15 and expires on December 31, 2011. Once the enrollment period is over, we will not be accepting any new enrollments until next year.
To enroll, you must either:
- have the Option payment waiting at the time of service
- send the Option payment in advance of your service
- call the office with a credit card authorization for the Option prior to service
For those of you who do not wish to enroll in an HMO, we will continue to offer you the same quality of service; however we do require that payment be provided at the time of service for all routine preventative care for your horse. Please be sure and ask us for the prices of the vaccines when you call for an appointment as they change occasionally.
